So that we can arrange costume, check any medical details, comply with council regulations and gain parental permission as the show will be filmed, we need you to complete this quick form below for all Stage Academy students by SUNDAY 4TH MAY 2025 at the very latest. One completed form per child please.

To help us arrange the correct costumers for the show, we need some basic information about your child's clothing size. Please remember, it is your responsibility to ensure your child brings the following basic items with them on show day; Stage Academy T-Shirt, Black T-Shirt, Black Bottoms (trousers/leggings/joggers), Black Shoes (ideally Jazz Shoes, however plimsoles, plain trainers or school shoes are fine as long as easy to move around in!), White Shirt, Black or White vest.

Medical Needs and Allergies
On show day children will need to bring a packed lunch/snacks with them. To help keep everyone safe it is really important that we know about any food allergies or medical needs. If your child has been at Stage Academy for a long time, their needs may have changed! Please take a moment to update us as appropriate.

- I agree to Stage Academy's "Photography and Filming" policy and give consent to the photography and filming of my child at Stage Academy activities. I understand and agree that photography and filming will take place at rehearsals and performances of the Stage Academy 2025 show and such pictures and recordings may be used in Stage Academy marketing materials in the future.
- I understand and agree that I must drop my child off at The Shaw Theatre, NW1 2AJ by the time above on the day of the performance and that I am responsible for purchasing tickets separately to watch the show.
